Functional Classification Systems for Children with Cerebral Palsy:An ICF-Based Approach

Read More Background: Cerebral palsy is one of the most prevalent physical disabilities in childhood. Children afflicted with this condition face a myriad of challenges and potential disabilities, which complicate treatment efforts. Integrating scales with other diagnostic instruments is increasingly crucial for accurately assessing these children and devising effective treatment strategies. This study primarily aims to review functional classification systems based on the International Classification of Functioning, Disability, and Health (ICF) for children with cerebral palsy and explore the relationships among these systems.methods: This study was designed to review the ICF-based functional classification systems. A comprehensive search was conducted across multiple databases, including Google Scholar, PubMed, ERIC, OVID, ProQuest, Scopus, Web of Knowledge, and OTseeker. Search terms related to cerebral palsy and various aspects of body function, activity, and participation were utilized. The search was conducted until September 2022 to gather relevant literature for analysis.Results: The analysis of the reviewed articles revealed the existence of five valid and reliable classification systems. In most studies, the correlation among these systems was moderate. Owing to their simplicity and comprehensiveness, these classification systems have significantly impacted the depiction of the functional status of children with cerebral palsy and the quality of their care. However, additional classification systems remain needed to address other overlooked functions and complete the overall description.Conclusion: The outcomes of this review indicated the development of classification systems for certain functions in children with cerebral palsy. In conjunction with the classifications above, creating additional new systems for overlooked functions could offer a comprehensive and integrated understanding of children with cerebral palsy's crucial functional capacities and performances.

Read More Background: Cerebral palsy (CP) affects different aspects of children's development. Some of these negative effects, such as behavioral issues in children, could be mitigated by teaching mothers, as the primary caregivers, the proper skills. The study aims to explore the impact of instructing communication skills to mothers as primary caregivers of children with cerebral palsy (CP) in mitigating behavioral problems exhibited by these children. Methods: In this semi-experimental study, we recruited 42 mothers with children aged 4-10 years diagnosed with cerebral palsy .The mothers were required to be literate, aged between 20 and 40, and their children's mobility score should be at or below 3 in the Gross Motor Function Classification System (GMFCS). Convenient sampling was used for participant selection. The Rutter Children's Behavioral Questionnaire was administered to assess the children's behavioral problems before and after the training. The experimental group participated in nine 90-minute training sessions to improve their communication with their children. In contrast, the control group received no intervention for children's behavioral problems. The collected data were analyzed using Multivariate Analysis of Covariance (MANCOVA). Results: Comparing the mean scores of the two groups in five sub-tests of the Rutter Children's Behavioral Questionnaire revealed significant differences in the following domains: Aggression & hyperactivity, Social incompatibility, Anti-social behaviors, Anxiety & Depression, and General Behavioral problems before the training, indicating variations between the groups (p = 0.001). Following the training, the experimental group significantly reduced all five subtests of Rutter's questionnaire (p = 0.001). Conclusion: The study suggests that equipping mothers with effective communication skills can lead to changes in their interactions with children who have CP and potentially reduce behavioral problems in these children. Consequently, rehabilitation teams may want to consider incorporating communication skills training programs into their overall therapeutic strategies for children with CP.

Thus, given importance of the role of language skills indevelopment of ToM and the previous research findings regarding the restrictedcapacity of children with cerebral palsy (CP) in development of their ability tounderstand others’ thoughts and feelings, this review study was designed tomainly evaluate the influence of language on ToM in children with CP.Methods: For this purpose, initially, electronic databases (PubMed, Scopus,PsycINFo, MEDLINE, Embase, and Web of Science) were searched fromSeptember 1 to 30 , during 2000-2020. Search was done focusing on the Englishwrittenpapers using a combination of keywords including: “Cerebral Palsy,Theory of Mind, Mentalization, and Language” to identify relevant studies from2000 to 2020.Results: A total of 978 publications were identified according to the initial searchcriteria. After reviewing abstracts, titles, and references of the identified papers,10 potentially relevant papers were selected. Based on assessing their full-text, 7papers completely met the inclusion criteria.Conclusion: Given that children with CP have not been addressed by numerousinvestigations to date, there is limited information about their ToM skills andthe role of other different components of ToM development. Therefore, furtherresearch should be carried out to investigate this issue. The combined findingsof the reviewed papers showed that language skills had an effect on capacity toelaborate ToM competence in children with CP. This review study paves the pathto the future common research on children with CP and underlines importanceof using a methodology, in which ToM performance is studied in concert with adetailed investigation of speech and motor impairments.2021© The Authors.
