Abstract

Background: Ataxia-telangiectasia, is a rare, progressive, and multi-system disorder and is the most prevalent autosomal recessive neurodegenerative ataxia. The most common rehabilitation interventions for them is occupational therapy. This study aims to show a top-down occupational therapy intervention based on the Occupational Therapy Practice Framework-4 (OTPF) in a 22-year-old girl with atypical ataxia-telangiectasia
Methods: The present study is a case report study, demonstrating the role of Top-down and client-centered occupational therapy interventions in a 22-year-old girl diagnosed with atypical ataxia-telangiectasia. This study was written using the CARE checklist according to the CARE Case Report Guidelines.
Results: Twelve sessions of occupational therapy based on OTPF resulted in improvements to the client's occupational performance, participation in activities of daily living, dynamic balance, and static balance. Canadian occupational Performance Measure mean performance score increased from 6.4 to 7.4, and satisfaction with performance score significantly increased from 5.4 to 8. Barthel Index score significantly increased from 90 to 100. Dynamic Gait Index score significantly increased from 9 to 17. Berg Balance Scale score significantly increased from 36 to 42.
Conclusion:
Occupational therapy intervention based on the OTPF-4 and emphasizing a top-down approach was described for a 22-year-old girl with atypical ataxia-telangiectasia. The interventions resulted in improvements to the client's occupational performance, participation in activities of daily living, and both static and dynamic balance.
